Starts Dec 1, 2011 or January 1, 2012. 
Minimum 6 months  or preferably One year assignment with renewals  as well as option for Permanent Full-time.
Care for Soldiers and their dependents spouses / family.  Air Force Base Clinic.  Requires any 50 state medical license.
Clinic hours M- F  8:00am – 4:30pm.  See 18 – 24 patients per day.
Call is 1:4  nights and weekends.
Hospital Rounding is 1:4 nights and weekends.  (approx.  0 -6 inpatients)  Some supervision and teaching of residents.
49 Weeks of Clinic, 3 weeks vacation (unpaid), 120 hours annually.
Federal Holidays off  (+/- 10 per year) 
Compensation package
Call 1:4 weekdays and weekend.  Hourly rate of $10.41 per hour (approx $250 / 24 hour). 
Base salary is paid on clinic hours at $87 per hour, not to exceed 40 per week.
All inpatient care hours and afterhours care are paid on a hourly basis from 0 -1128 hours per year at $77.00 / hour.

Minimum earnings:  $ 213,000   Estimated Earnings Average:  $ 222,200+  Maximum 1^st year Earnings:  $  263,760  (max amount allowable with overtime / inpatient care).

All Professional Liability Insurance provided.  Comp is based on weekly timesheet and the actual hours served.  Any termination by physician requires 90 days notice. 

A one-time relocation reimbursement of $5,000 provided to be paid by government within 90 days of start date.
No stipends or separate travel expenses provided. 

Must be  board certified in Internal Medicine or Family Practice or board eligible within 5 years of residency

**Dates: January 3, 2012 onward ( flexibility with dates and people who only have extremely limited availability over the next 6 months) at 49 bed hospital with 2 general surgeons, 2 orthopedic surgeons, 5 Family Practitioners,  2 Internists , Emergency Medicine Dept, 3 respiratory therapists on staff

** Must have MT license or very close to getting a MT license

**Must have a residency in Family Practice or Internal Medicine

**Must have an interest in doing inpatient work that includes mechanical ventilator management and ICU Management (central lines and intubation not required)

** Will be working 8 hours shifts Monday –Friday with shared call during weekends( primarily phone call….do not have to stay at hospital)

 **May be potential for permanent opportunity  

**Patient volume: 1 to 3 patients per weekday on average, 1 to 6 patients per weekend day on average

**Apartment  3 minutes from hospital provided . As needed , rental car and airfare provided as well.

** Board Certification is not required

When on call, physician will be taking care of impatients as well as back up call for ED.

Medical Malpractice Coverage procured through group policy.

4 weekdays per week  ongoing (could turn into permanent possibility)

Must be licensed in Illinois or currently being processed

Patient volume: approximately 15 to 20 patients per day

2 different locations

No weekend work or call

Medical Assistants at each site

Board certification or eligibility not required
